Our facility has two towers—the Central Tower and the East Tower. Centrum Surgical Center is on the third floor of the EAST TOWER, which is further north, taller, and rounded. Please make note, as it is not the first building people see when they first park.
On the day of your surgery, please arrive at the center at the time given to you by the preoperative nurse during your phone call. Typically, this will be at least one and a half hours ahead of your scheduled surgery time, but it may vary depending on physician requests.
Our friendly business specialists will ask you to check in at the Registration Desk and complete the necessary paperwork for your procedure, including a “Consent for Treatment” form acknowledging your permission for the physicians and facility to care for you. If the patient is a minor, the legal guardian will complete the necessary paperwork and be responsible for staying in the center.

The pre-surgical assessment should have been completed before your arrival. If it hasn’t been completed, you will be requested to complete it before preparing for your procedure. *Please fill it out before your arrival so our anesthesia team can review it beforehand. *
Once the paperwork is complete, a nurse will bring you back to the preoperative area, where you will get ready for your surgery with our clinical team. Throughout the process, we will keep you updated with any possible delays.
In the preoperative area, your surgeon will see you, complete any necessary last-minute paperwork, and answer any questions. If you are having anesthesia, you will meet with your anesthesia provider, have an opportunity to ask questions and complete the necessary paperwork. The length of your procedure will vary based on what you have done.
